Admissions Forms

If the child you referred is accepted for placement at Southern Peaks, a representative of our admissions department will work with you and our Southern Peaks program staff to schedule an admission date and coordinate any necessary pre-admission documentation (e.g. consents, authorizations, Interstate Compact approval).

After acceptance and prior to admission, we will ask that you ensure the following documents are completed, signed, and returned to Southern Peaks:
